To be eligible for the Phil Guza Memorial Scholarship an individual must be a graduating high school senior pursuing a bachelor’s degree in math or science. Although not a requirement, preference will be given to students of Lincoln High School in Philadelphia, PA (Phil’s alma mater).

The 2017 Application Form for the Phil Guza Memorial Scholarship is available for download below. All aspects of the application require postmark no later than May 5, 2017, and applications will not be reviewed until all required material has been received. The Charity will award a total of $1,750 in grants this year.

To apply for the Scholarship a student is required to submit the following:
  • A basic questionnaire outlining the applicant’s personal information and anticipated higher education. Below is the 2016 Application Form

  • High school transcript
  • SAT/ACT scores (only one or the other is required)
  • Outline of extracurricular involvement (work experience, volunteering, academic clubs, sports, leadership positions, etc)
  • One letter of recommendation from a high school teacher, coach, guidance counselor, or other qualified authority
  • An essay of approximately 500 words addressing one of the two topics below:
    • Why you’re interested in pursuing a higher education in math or science
    • What you’re most passionate about and why

An application will not be reviewed until all above criteria are received.
Submission due date requires postmark no later than May 5, 2017.
The recipients will be contacted and announced by June 30, 2017.

The Executive Director will review all complete applications received by the deadline and submit a set of finalists to the Selection Committee (the remainder of the Board of Trustees), who will collectively choose the beneficiaries. Verification of college enrollment must be provided prior to disbursement, which will make payment directly to the student and is intended to be used specifically on college school supplies such as tuition, room and board, or textbooks.
